This is the application form for the
Scottish Assembly of Parents and Carers. You can find more information about
the Assembly here.
We welcome applications from parents and
carers of school-aged children in Scotland.
You must submit answers to questions which
have a red asterisk. This application asks for:
Your contact details – so we can get in
touch with you.
To make sure the Assembly is
representative of the diversity of Scotland’s parents and carers we also ask
for:
- Information about you
- Information about your child/children and their school setting
- Your preferred communication methods
There is a comment box where you can expand
on your answers or add anything you think we should know, should you wish to.
We would also like to know what areas around Scottish education are of interest to you.
